T.E.A.M. – Technology Enhanced Aftercare & Monitoring

A functional aftercare program designed and offered by Sovereign Health of California.

Aftercare:

The support and contribution rendered to individuals recovering from addiction towards their commitment to sobriety. More than half the client population is prone towards relapse even within the first year of successful completion of transitional living. This shows the significance of post recovery Aftercare and Follow-up programs for such candidates to participate and continue a life of long term sobriety.

After care programs involve assisting the recovering individual in returning to normal life, dealing with stress, finding a healthy social group and resisting the craving for the drug/ alcohol/ narcotic. Recovering individuals are encouraged to take part in support groups or 12-step programs. Individual therapy may also be continued throughout the aftercare process. If drug use was attributed to trauma or mood disorder, counseling and/or medications may need to be continued for several months or even years.
